First Responder's Scholarship Application
PURPOSE
In cooperation with the Education Foundation of Kenosha, the Proko Funeral Home is sponsoring a $1,500.00, one-time scholarship to provide assistance to a Kenosha student who will be graduating this year.
ELIGIBILITY
The student must be a graduating senior and:
Have a parent or parents that serve the Kenosha community as a first responder (police, firefighter, EMT)
Demonstrate strong community-centered values and a will be pursuing a 4-year college or technical school education
Participate in extracurricular activities or church/community service
SCHOLARSHIP REQUIREMENTS
Present one letter of recommendation from a current teacher, current principal and/or community leader.
Submit an essay explaining:
What you have learned from your parent’s service.
One thing you would like the community to know about your first responder parent/parents.
What is your chosen career path and why (no more than 200 words).
FINANCIAL DATA
The scholarship award amount is $1,500. The award is to be used for tuition at an accredited college or university, and will be issued directly to the educational institution. Payment of scholarship will be made to university/college after one completed semester. Students must provide transcripts upon completion of first semester.

Complete this application and submit by March 15th.
