Mroz-Chernik Memorial Scholarship Application

PURPOSE
In cooperation with the Education Foundation of Kenosha, the Mroz-Chernik family is sponsoring a $1000.00, one-timescholarship to provide assistance to a female Kenosha Unified School District student who will be graduating this year and pursuing a degree in education to pursue a teaching certificate. 

ELIGIBILITY
Interested students must be a female graduating from a Kenosha Unified high school and pursuing a degree in education to obtain a teaching certificate. Academically capable students must demonstrate a financial need, provide a transcript showing a 3.0 grade point average or better, and be a U.S. citizen. The scholarship may be used only at an accredited institution of higher education located in the United States of America. 

FINANCIAL DATA
The scholarship award amount is $1000. The award is to be used for tuition, fees, and college residential costs at an accredited college or university, and will be issued directly to the university.  Payment of scholarship will be made to university/college after one completed semester. Students must provide transcripts upon completion of first semester.
